Transforming Youth Care: Continuation of Care Recommended Guidelines

This project aims to create a policy framework titled "Continuation of Care Recommended Guidelines" for youth transitioning into adulthood, focusing on ages approximately 12 to 24. 

Central to these guidelines is the establishment of clear definitions for youth, the inclusion of a grace period post-aging out, and the addition of those often overlooked due to perceived 'lack of severity'. The framework emphasizes immediate feedback through the Integrated Youth Services (IYS), categorizes varying service levels, and mandates severity classifications. Recognizing diverse needs, the project advocates for resource mapping, cultural sensitivity training, and ensuring physical accessibility. It also seeks to foster partnerships, especially with Indigenous communities, and integrate insights from diverse perspectives. Through this initiative, we aim to create a pan-Canadian IYS blueprint, urging both governmental and private sector collaboration. These guidelines will be published in a pamphlet, which will be available to all Canadian IYS in February.
